How do I sign up?
Email firstname.lastname@example.org and request access to the Patient Portal. You’ll receive an email confirmation with instructions for activating your account online.
How do I access the Cooper Clinic Patient Portal?
Once you have activated your account online, visit mycooper-clinic.followmyhealth.com and log in using your Patient Portal username and password. You can also access the portal on your smart phone or tablet with the FollowMyHealth app available for free on iTunes and Google Play.
